Crossroads is a member of the award-winning Connected Palliative Care Team – a specialist end of life care service in Sandwell. Connected Palliative Care support people wishing to live in their own home to the end of their life and provides access to a palliative care consultant and clinically trained staff, delivering 24/7 care. Through Connected Palliative Care, we can also offer respite care for family carers as they care for their loved one.
