Accessibility details | Eligibility: - Aged 50 and over - Sandwell resident or registered with a GP in Sandwell Inclusion Criteria (1 or more of the following): - Fear of Falling - Feeling unstable - History of Falls (injurious and non-injurious) - Low bone density and/or family history of osteoporotic fracture. - Be able to stand independently with/without walking aid - Be able to mobilise independently with/without walking aid Exclusions: - GP has said that you have a heart condition and has recommended only medically-approved physical activity. - Pain - Angina - Acute systemic illness (cancer) - Visual or vestibular disturbances - Significant drop in BP during exercise - Unstable or acute heart failure - Unable to maintain seated upright posture - Impaired cognition - Diagnosis of dementia |

The Sandwell Falls Prevention Service is aimed towards those aged 50+ who struggle with mobility or balance, registered with a Sandwell GP or living in Sandwell, including those who are at risk or are fearful of falling as well as if any falls have already happened. Our service aims to help older adults improve their mobility, strength and balance to prevent the risk of having falls as well as reducing the risk or severity of health conditions related to inactivity. Individuals can be referred to us through their GP, self-referral via our website or via call/email. Once we have received a referral, our team will be in touch with the client to arrange an assessment. This helps us to understand their circumstances and level of mobility and balance. This includes: - A basic questionnaire to help us understand their health and circumstances - A falls risk assessment - A mental wellbeing assessment - A short walk (Timed Up and Go) test This allows us to calculate the risk of falling and provide clients with the necessary support through either our 12 week Chair Based Falls Prevention Exercise Programme led by a qualified instructor or the complimentary workbook to enable them to exercise at home if they are unable to attend. Our team will also provide clients with Information, Advice and Guidance on ways to prevent falls and exercise safely at home. Another useful resource for further information is our 'Staying Steady' guide which can be found on the AgeUK Sandwell website is free to download and provides information and tips on how to reduce the risk of falling.

We offer a comprehensive range of support, including: • Information and Advice • Support for those living with dementia and their carers • Social Groups • Physical activities and exercise groups • A Wills and Lasting Powers of Attorney Service. • Computer Clubs to help people become more digitally confident We also offer: • talks in the community to target key health issues • hold events and activities to raise awareness of health campaigns and local health initiatives • develop and maintain partnerships with other voluntary and statutory organisations to ensure services are developed and promoted in co-ordination. Age UK Sandwell relies on the support of dedicated volunteers to help deliver our services.
