The Sandwell Falls Prevention Service supports individuals aged 50+ who have difficulties with mobility or balance. It is available to those registered with a Sandwell GP or living in Sandwell, including people at risk of falling, those who are fearful of falling, or those who have already experienced a fall. The service aims to help older adults improve mobility, strength and balance, reducing the likelihood of falls and the risk or impact of health conditions linked to inactivity. Referrals can be made through a GP, by self-referral via the website, or by phone or email. Once a referral is received, the team contacts the individual to arrange an assessment. This includes a questionnaire to understand health and circumstances, a falls risk assessment, a mental wellbeing check, and a short walking test (Timed Up and Go).
